How We Can Work Together To Make Your First Visit A Great One.

The professionals at West Coast Dental affiliated practices are pleased that you have considered us to take care of your smile and your dental needs. Your first visit will probably include a thorough dental exam and x-rays as well as a consultation to discuss your smile and your dental health with you. If you have some immediate needs, they may also be addressed. We want your first and every visit with us to be as pleasant and successful as possible.

1. Bring any dental records or X-rays that you might have
2. Bring in your completed Patient Information Form, Smile Evaluation Form and Health History Form. Please click on following links to download appropriate forms. Patient Information – English or Patient Information - Spanish and Smile Evaluation Form –English or Smile Evaluation Form – Spanish and lastly, the Health History form. 

   or

   Bring your insurance information, information about medications you are taking and your health history with you and plan to arrive 15 minutes early to fill out the information at our office.

3. Prepare a list of any questions that you might want to ask

We recommend that you call us before your first appointment if you have any questions or to find out if there is any additional information we might need.

Patients under the age of 18 must be accompanied by a parent or guardian on their first appointment.
